Order Form PROPER format.

I have extended the deadline of the sale to 10th July, Tuesday. 


Many of you who sent in the orders had the format wrong and I had to email you one by one to confirm and clarify your orders..
So here's a post on how to fill in the order form in the proper format so I don't get your orders wrong.

Once you've clicked on the link to the Order Form, you will see this: (click to view in full size)

Send in your orders here:

Your Personal Information: 

1. Fill in your name, (Full name please!)
2. Contact Number, (Handphone Number, we need to send out a confirmation SMS after the closing date)
3. Address, WITH POSTAL CODE. (Just in case you can't make it for the collection, I'll mail the item to you!)
4. Email Address, (Optional but I would prefer you left it)

Your Orders: 

1. Include the Item Name and Code.
E.g: #3 Running Man Logo Shirt 

2. State Quantity & Color.
E.g: #3 Running Man Logo Shirt, (1, Black) 

3. State Size. Include if it's Mens' OR Womens' Size.
E.g: #3 Running Man Logo Shirt, (1, Black, Womens' Size S)

So, your completed order should look like this: (An example)
#3 Running Man Logo Shirt, (1, Black, Womens' Size S)

For those who have already sent in your orders and realized you left out some information,
DO NOT SEND IN YOUR ORDERS AGAIN. 
Check your emails, I have sent some emails to confirm your information & orders.
If I do not get a reply from you before the closing date, your orders will be automatically cancelled.
